The applicant states in effect: a. She was married on 24 August 2019 and needs to have her records reflect her new last name of Z. Presently, her retired pay is still in the name Y and needs to be changed to her married name, Z. Her marriage occurred on 24 August 2019. b. She updated her information in the Defense Enrollment Eligibility Reporting System (DEERS) at a local Army Reserve Center on 19 September 2019 to reflect her new married name. She thought this would update the name on her Army retired pay, but she found out in early October of 2019, that the name had not been updated in the Defense Finance Accounting Service (DFAS) system. She thinks her name is correct in DEERS, but it would need to be updated for her retired pay. 2. In reference to the portion of the applicant’s request to change her last name from Y to Z on her retired pay records, DFAS is the appropriate agency for this action. The applicant may contact DFAS to assist with her request. DFAS is the appropriate agency for correcting the applicant’s Retiree Account Statement. DFAS customer care center (DFAS CCC) can be contacted at dfas.cleveland-oh.hi.mbx.investigation- hotline@mail.mil. This portion of her request will not be addressed any further in the record of proceedings. The Board will consider her request for correction of her service records to reflect the name Z. 3. A review of the applicant’s military service records shows: a. On 27 September 1978, she enlisted in the Regular Army. b. On 15 September 1980, she requested a name change to Y due to marriage, effective 6 September 1980. c. On 28 March 1988, she was honorably released from active duty by reason of reduction in authorized strength in Fiscal Year 1988. d. On 29 March 1988, she enlisted in the US Army Reserve (USAR). e. Orders Number D-03-017100 issued by the US Army Reserve Personnel Command, dated 21 March 2000, honorably discharged her from the USAR. Her last name at the time is listed as Y, consistent with all of her service records. 4. The applicant provides: a. Marriage Certificate, which shows she got married on 24 August 2019, after her military service. b. A copy of her US Army Retired ID Card and State of Driver’s License, which reflect her married last name of Z. 5. See applicable references below. BOARD DISCUSSION: 1. After reviewing the application and all supporting documents, the Board found that relief is not warranted. 2. The Board agreed that the applicant's post-service name change is an insufficient basis for changing the name recorded in her service records. The Board determined there is no error or injustice in this case. 3. The applicant is advised that a copy of these proceedings will be filed in her service record, which should serve to explain the difference between the name in her service record and the name she now uses. REFERENCES: Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. The regulation directs that the purpose of the separation document is to provide the individual with documentary evidence of his or her military service. It provides a brief, clear-cut record of active duty service at the time of release from active duty, retirement, or discharge.
