The applicant states he was notified in 2019 that he received an Army Superior Unit Award from his time serving on the United States Military Academy staff from 2013 to 2016. He would like to update his DD Form 214, Officer Record Brief, and iPERMS, to reflect this award on his list of awards. This correction should be made so that his records accurately show all the awards he received while on active duty. 2. Review of the applicant's service records shows: a. He was appointed as a Regular Army commissioned officer and executed an oath of office on 28 May 2005. He was promoted to major in September 2015. b. He was assigned to Headquarters, Staff and Faculty Company, U.S. Military Academy around June 2013. He performed duties of Instructor from April 2015 to April 2015 and Executive Officer from April 2016 to July 2016. c. On 5 October 2016, Headquarters, U.S. Army Garrison, West Point, NY published orders discharging him from active duty on 1 October 2016. d. His DD Form 214, as amended by a DD Form 215, shows he was discharged on 1 October 2016, having completed 11 years, 4 months, and 4 days of active duty. It also shows he was awarded or authorized: * Iraq Campaign Medal with 2 campaigns stars * Bronze Star Medal * Army Commendation Medal (2nd Award) * Army Achievement Medal (2nd Award) * Meritorious Unit Commendation * National Defense Service Medal * Global War on Terrorism Service Medal * Army Service Ribbon * Overseas Service Ribbon * Sapper Tab * Ranger Tab * Parachute Badge * Air Assault Badge e. On 2 October 2016, he was appointed as a Reserve commissioned officer of the Army. He is currently serving in the USAR. 3. Permanent Orders 311-008 issued by the U.S. Army Human Resources Command on 8 November 2018 awarded the Staff and faculty, U.S. Military Academy the Army Superior Unit Award for meritorious performance of service from 1 July 2015 to 31 January 2017. 4. By regulation (AR 635-8), the information entered on the DD Form 214 reflects the conditions as they existed at the time of separation. The instruction for Block 13 (Decorations, Medals, Badges, Citation and Campaign Ribbons Awarded or Authorized) is to list all federally recognized awards and decorations for all periods of service. 5. A review of the applicant's AMHRR shows a copy of Permanent Orders 311-008 was filed in the applicant's AMHRR on 29 June 2021. The naming convention AMHRR is an umbrella term encompassing human resource records for Soldiers, retirees, veterans, and deceased personnel. The AMHRR includes, but is not limited to, the Official Military Personnel File (OMPF), finance related documents, and non-service related documents deemed necessary to store by the Army. The OMPF is a collection of information which permanently documents a Soldier's career in the military. The OMPF contains documentation pertaining to accession, training, education, assignment, performance, discipline, decoration, casualty and separation of the service member. The AMHRR is archived in iPERMS which has been identified as a Records Management Application for the Army. 6. The Officer Record Brief (ORB) is a one-page tabular display of an officer's career history, qualifications, education, and experience. The ORB is an important tool used in the management of officers. Data on the ORB reflects events concerning an officer's military career. These events are routinely updated or reported by the unit S-1, the MPD (Military Personnel Division), or other HR agencies. The record brief is reviewed, in conjunction with the Record Manager, at least annually as part of the personnel records review in iPERMS. BOARD DISCUSSION: After reviewing the application, all supporting documents, and the evidence found within the military record, the Board determined that relief was warranted. The Board carefully considered applicant’s contentions, military record, and regulatory guidance. Documentation available for review shows a permanent order designating the United States Military Academy one of several military academies which received the unit award. The applicant was assigned to the United States Military Academy during the time USMA was awarded the Army Superior Unit Award. Based on the preponderance of evidence available for review, the Board determined the evidence presented sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending block 5 of his DD Form 215 for the period ending 1 October 2016 to show the award of the Army Superior Unit Award for the period 1 July 2015 – 31 January 2017. Army Regulation 635-8 (Separation Processing and Documents) prescribes the separation documents that must be prepared for Soldiers upon retirement, discharge, or release from active duty service or control of the Active Army. It established standardized policy for preparing and distributing the DD Form 214. The DD Form 214 is a summary of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of all current active, prior active, and prior inactive duty service at the time of release from active duty, retirement, or discharge. The information entered thereon reflects the conditions as they existed at the time of separation. The instruction for Block 13 (Decorations, Medals, Badges, Citation and Campaign Ribbons Awarded or Authorized) is to list all federally recognized awards and decorations for all periods of service. Do not use abbreviations. Do not enter foreign or State level awards. 2. Army Regulation 600-8-104 (Army Military Human Resource Records Management) prescribes Army policy for the creation, utilization, administration, maintenance, and disposition of the Army Military Human Resource Record (AMHRR). The naming convention AMHRR is an umbrella term encompassing human resource (HR) records for Soldiers, retirees, veterans, and deceased personnel. The AMHRR includes, but is not limited to, the official military personnel file (OMPF), finance related documents, and non-service related documents deemed necessary to store by the Army.
