The applicant enlisted into the Regular Army on 13 April 2006. Item 1 (NAME (Last, First, Middle)) of her DD Form 4 (Enlistment/Reenlistment Document – Armed Forces of the United States) lists a contested name. 2. The applicant was honorably discharged on 31 August 2011. Her record contains a DD Form 214 (Certificate of Release or Discharge from Active Duty), which lists the contested name. 3. The applicant provides a copy of an Order for Individual Name Change, which legally changed her name as she requests, effective 3 August 2020. 4. The applicant's military record did not contain any documentation that shows the name she now requests. 5. The DD Form 214 is a synopsis of the Soldier's most recent period of continuous active service. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. BOARD DISCUSSION: After reviewing the application and all supporting documents, the Board determined relief was not warranted. Based upon the available documentation showing the applicant changed her name post-service and the DD Form 214 responsible for reflecting correct information at the time of its publication, the Board concluded there was insufficient evidence of an error or injustice which would warrant a change to the applicant name reflected on the DD Form 214.
