The applicant states his name has changed twice since leaving active service. He claims it is very difficult explaining the different name without several pages of documentation each time he has to provide his DD Form 214. 2. The applicant enlisted into the Regular Army on 8 April 2009. Item 1 (NAME (Last, First, Middle)) of his DD Form 4 (Enlistment/Reenlistment Document - Armed Forces of the United States) lists a contested name. 3. The applicant was honorably released from active duty on 5 October 2015. His record contains a DD Form 214 which lists the contested name. 4. The applicant provides a copy of an Order from a Circuit Court, which legally changed his name as he requests effective 9 October 2020. 5. The applicant's military record did not contain any documentation that shows the name he now requests. 6. The DD Form 214 is a synopsis of the Soldier's most recent period of continuous active service. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. BOARD DISCUSSION: 1. After reviewing the application, all supporting documents, and the evidence found within the military record, the Board found that relief was not warranted. The applicant’s contentions, the military record, and regulatory guidance were carefully considered. Regulatory guidance provides the DD Form 214 shows the circumstances that existed at the time the form was created. Evidence of record shows the applicant used the contested name during his entire period of service. Therefore, the Board determined the overall merits of this case are insufficient as a basis for correction of the records of the individual concerned. 2. The applicant is advised that a copy of this decisional document, along with his application and the supporting evidence he provided, will be filed in his official military records. This should serve to clarify any questions or confusion regarding the difference in his name recorded in his military records and to satisfy his desire to have his new name documented in his military records.
