IN THE CASE OF: BOARD DATE: 25 February 2022 DOCKET NUMBER: AR20210010311 APPLICANT REQUESTS: correction of the social security number (SSN) shown on his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) from A (xx5-xx-xxxx) to B (xx9-xx-xxxx). APPLICANT'S SUPPORTING DOCUMENTS CONSIDERED BY THE BOARD: * (DD Form 149 (Application for Correction of Military Record), 10 September 2020 * Copy of Driver’s License * Copy of Medicare Health Insurance FACTS: 1. The applicant did not file within the three-year time frame provided in Title 10, United States Code, section 1552(b); however, the Army Board for Correction of Military Records (ABCMR) conducted a substantive review of this case and determined it is in the interest of justice to excuse the applicant's failure to timely file. 2. The applicant states that his social security number needs to be corrected from (A) to the correct one (B). He did not know how to get it corrected until now. 3. On 15 October 1968, the applicant enlisted in the Regular Army. 4. His DD Form 4 (Enlistment Record), is not available for review. 5. The applicant provided his driver’s license that shows the same name and date of birth listed on the DD Form 214 and his Medical Health Insurance Card; however, neither shows his SSN. 6. On 20 July 1971, he was honorably released from active duty. His DD Form 214 is not fully legible, but shows he completed 2 years, 9 months and 6 days of net service. His DD Form 214 shows in item 3 (Social Security Number) – his SSN as A. 7. By regulation (AR 635-8), the DD Form 214 is a summary of the Soldier's most recent period of continuous active duty. The information entered thereon reflects the conditions as they existed at the time of separation. For item 3, verify accuracy by reviewing initial enlistment contract and/or application for appointment. If the Soldier has had more than one SSN, list the other SSN of record in block 18. BOARD DISCUSSION: 1. After reviewing the application, all supporting documents, and the evidence found within the military record, the Board found that relief was/was not warranted. The applicant’s contentions, the military record, and regulatory guidance were carefully considered. Evidence of his SSN within his record other than the DD Form 214, which he authenticated with his signature, was not available for the Board to review and hid did not provide any evidence showing the SSN he requested belongs to him. The Board determined there is insufficient evidence to grant relief. 2. The applicant is advised that a copy of this decisional document, along with his application and the supporting evidence he provided, will be filed in his official military records. This should serve to clarify any questions or confusion regarding the difference in the SSN recorded in his military records and to satisfy his desire to have the SSN he requested documented in his military records.
