FACTS:

1.  The applicant enlisted into the Regular Army on 20 January 1994.  Item 1 (NAME 
(Last, First, Middle)) of her DD Form 4 (Enlistment/Reenlistment Document - Armed 
Forces of the United States) lists the requested name. 

2.  The applicant retired honorably on 30 September 2014.  Her record contains a 
DD Form 214 (Certificate of Release or Discharge from Active Duty), which lists the 
contested name.

3.  The applicant provides a copy of a Final Decree of Divorce which changed her name 
as she requests effective 9 July 2019.  She also provides copies of her Driver License, 
Social Security Card and U.S Uniformed Service Identification Card which lists her 
name as she requests.

4.  The DD Form 214 is a synopsis of the Soldier's most recent period of continuous 
active service.  It provides a brief, clear-cut record of active Army service at the time of 
release from active duty, retirement, or discharge.  





BOARD DISCUSSION:

After reviewing the application, all supporting documents, and the evidence found within 
the military record, the Board found that relief was not warranted.  The applicant’s 
contentions, the military record, and regulatory guidance were carefully considered.  
Regulatory guidance provides the DD Form 214 shows the circumstances that existed 
at the time the form was created.  It also provides that previously used names would be 
reflected in the Remarks section of the form.  Her DD Form 214 properly lists her 
previously used name in the Remarks section, as she separated from service using the 
contested name.  The Board determined there was no error or injustice in this case.

The evidence presented does not demonstrate the existence of a probable error or 
injustice.  Therefore, the Board determined the overall merits of this case are insufficient

the Soldier's most recent period of continuous active service.  It provides a brief, clear-
cut record of active Army service at the time of release from active duty, retirement, or 
discharge.  Block 1 (NAME (Last, First, Middle)) states to compare the original 
enlistment contract or appointment order and review the official record for possible 
name changes.  If a name change has occurred, list other names of record in block 18 
(Remarks).
