The applicant states: a. His DD Form 214 (Certificate of Release or Discharge from Active Duty) should be corrected to show in item 18 (Remarks): * Service in Kuwait from 7 January 2019 to 5 May 2019 * Service in Syria from 18 January 2019 to 19 February 2019 * Service in Kuwait from 20 February 2019 to 5 May 2019 b. The correction should reflect his locations while deployed in support of Operation Inherent Resolve. 2. On a date unknown, the applicant enlisted in the U.S. Army Reserve, and on 9 November 2018, he was ordered to active duty in support of Operation Inherent Resolve. 3. The applicant provides the following documents: a. Orders Number BL-358-0058, issued by Headquarters, U.S. Army Garrison, Fort Bliss, 24 December 2018, which shows, in part, temporary change of station orders – * Assigned to: 779th Engineer Company * Purpose: Deployment will be in support of Operation Inherent Resolve, Camp Buerhing, Kuwait * Will proceed on or about: 1 January 2019 b. Several Defense Finance and Accounting Service (DFAS) Military Leave and Earnings Statement (LES) which shows, in part – (1) LES, dated 23 January 2019, shows in the remark section applicant received Imminent Danger Pay/Hardship Duty Pay from 8 January 2019 to 15 January 2019. (2) LES, dated 1 February 2019, shows in the remark section applicant received Imminent Danger Pay/Hardship Duty Pay from 16 January 2019 to 31 January 2019. (3) LES, dated 15 February 2019, shows in the remark section applicant received Imminent Danger Pay/Hardship Duty Pay from 1 February 2019 to 15 February 2019. (4) LES, dated 1 March 2019, shows in the remark section applicant received Imminent Danger Pay/Hardship Duty Pay from 16 February 2019 to 30 February 2019. c. Orders Number BL-358-0058 (A2), issued by U.S. Army Garrison, Fort Bliss, 11 March 2019, which shows, in part, amendment to Order Number BL-358-0058, dated 24 December 2018, changed to: Soldier will also perform duty at Iraq and Syria. d. LES, dated 1 April 2019, shows in the remark section applicant received several collections for – * Imminent Danger Pay from 8 January to 18 January 2019, 20 February to 28 February 2019, and 1 March to 15 March 2019 * Hardship Duty Pay from 20 February to 28 February 2019, 1 March to 15 March 2019, and 16 March to 31 March 2019 e. A DA Form 2173 (Statement of Medical Examination and Duty Status), 13 April 2019, which shows in part, on 13 February 2019, the applicant was deployed to Syria, and injured his knee while stacking HESCO barriers. 4. On 4 July 2019, the applicant was honorably released from active duty. His DD Form 214 shows he completed 7 months and 26 days of service. It also shows in: * item 12f (Foreign Service), 3 months and 29 days * item 18, in part, SERVICE IN KUWAIT 20190107-20190505 5. The applicant provides – a. A Defense Accounting Systems report prepared on 4 January 2022, which shows the applicants active-duty status from January to July 2019. b. A memorandum for record, issued by Captain 23 June 2022, which shows, in part, the [applicant] served as a team member in his platoon and deployed with the 779th Engineer Company in January 2019. During the deployment [applicant] served in Syria between the dates of 18 January to 19 February 2019. BOARD DISCUSSION: After reviewing the application, all supporting documents, and the evidence found within the military record, the Board found that relief was warranted. The Board carefully considered the applicant's record of service, documents submitted in support of the petition and executed a comprehensive and standard review based on law, policy and regulation. Upon review of the applicant’s petition and available military records the Board determined the governing regulation provides that at separation the service member’s record will be used to enter accurate information when completing their DD Form 214. The Board determined the applicant’s DD Form 214 in item 18 (Remarks) is absent his three deployment periods in support of Operation Inherent Resolve to Kuwait for the periods of 7 January 2019 until 5 May 2019 and 20 February 2019 until 5 May 2019 as well as his deployment to Syria for the period of 18 January 2019 until19 February 2019. Based on this, the Board granted relief to amend the applicant’s DD Form 214 to accurately reflect his deployment period.
