Evidence of record shows that on 5 December 1996, DA, HQ, 101th Mountain Division (Light Infantry) and Fort Drum, New York, Orders 340-1020 discharged the applicant from the Regular Army, effective 13 December 1996. The record contains a properly constituted DD Form 214 (Certificate of Release or Discharge from Active Duty). His DD Form 214 indicates that he was discharged under the provisions of Chapter 13, AR 635-200 by reason of unsatisfactory performance, with a characterization of service of general, under honorable conditions. The specific facts and circumstances pertaining to the applicant’s discharge are not contained in the available records. b. On 13 December 1996, the applicant was discharged.
