Facts and Circumstances: Evidence of record shows that on 21 October 1994, the unit commander notified the applicant of initiation of separation action under the provisions of Chapter 5, AR 135-178, by reason of failure to ship for initial active duty for training/apathy. The specific facts and circumstances leading to the applicant’s discharge from the United States Army Reserve are not contained in the available records.
