Pursuant to the provisions of reference (a), Petitioner, an enlisted member of the Marine Corps, filed enclosure (1) with this Board requesting that his naval record be corrected by removing a 6105 Page 11 entry dated 1 April 2013 from his record. This request includes, but is not limited to, removal of alt references to the Page 11 entries from his Official Military Personnel File (OMPF), the Marine Corps Total Force System (MCTFS), and his Electronic Service Record (ESR). Enclosures (1) through (4) apply. 2. The Board, consisting of , reviewed Petitioner's allegations oferror and injustice on 16 August 2016 and, pursuant to its regulations, determined that the corrective action indicated below should be taken on the available evidence of record. Documentary material considered by the Board consisted of the enclosures, naval records, and applicable statutes, regulations, and policies. In addition, the Board considered the advisory opinion (AO) furnished by Manpower Information Quality Assurance Division (MIQ), dated 1 July 2016, copies ofwhich are attached as enclosure (3). 3. The Board, having reviewed all the facts ofrecord pertaining to Petitioner's allegations of error and injustice finds as follows: a. Before applying to this Board, Petitioner exhausted all administrative remedies available under existing law and regulations within the Department ofthe Navy. b. Enclosure (1) was filed in a timely manner. c. On 1 April 2013, Petitioner was counseled for failing to discharge the duties as EKMS Manager by not completing a required inventory in August 2012. e. On 16 April 2013, Petitioner was informed that the Page 11dated1April2013 contained multiple errors and did not include his rebuttal statement. After signing the revised Page 11, Petitioner was informed that the corrected Page 11 and rebuttal statement would be uploaded in his OMPF. CONCLUSION: Upon review and consideration of all the evidence of record, and especially in light of the 6105 Page 11 entry dated 1 April 2013, the Board concluded that there is an existence ofan injustice, and as such, Petitioner's request .warrants favorable action. In this regard, the Board substantially concluded that the 6105 Page 11 entry dated 1 August 2013 should be removed from Petitioner's OMPF. RECOMMENDATION: a. That Petitioner's naval record be corrected by obliterating or removing the 6105 Page 11 entry dated 1 April 2013 from his OMPF. b. That any material or entries inconsistent with or relating to the Board's recommendation be corrected, removed, or completely expunged from Petitioner's record and that no such entries or material be added to the record in the future. 4. Pursuant to Section 6(c) of the revised Procedures ofthe Board for Correction ofNaval Records (32 Code of Federal Regulations, Section 723.6(c)), it is certified that a quorum was present at the Board's review and deliberations, and that the foregoing is a true and complete record ofthe Board's proceedings in the above titled matter. 5. Pursuant to the delegation of authority set out in Section 6( e) ofthe revised Procedures ofthe Board for Correction ofNaval Records (32 Code ofFederal Regulation, Section 723.6(e)) and having assured compliance with its provisions, it is hereby announced that the foregoing corrective action, taken under the authority ofreference (a), has been approved by the Board on behalf ofthe Secretary of the Navy.
