Pursuant to the provisions of reference (a), Petitioner, an enlisted member of the Navy, filed enclosure (1) with this Board requesting her Physical Readiness Information Management System (PRIMS) record be corrected for Cycle 2, 2013 Physical Fitness Assessment (PFA). 2. The Board, consisting of , reviewed Petitioner's allegations of error and injustice on 8 August 2018 and, pursuant to its regulations, determined that the corrective action indicated below should be taken on the available evidence of record. Documentary material considered by the Board consisted of the enclosures, relevant portions of Petitioner's naval records, and applicable statutes, regulations and policies. The Board also considered the advisory opinion (AO) provided by the Chief of Naval Operations (OPNAV Nl70). 3. The Board, having reviewed all the facts of record pertaining to Petitioner's allegations of error and injustice finds as follows: a. Before applying to this Board, Petitioner exhausted all administrative remedies available under existing law and regulations within the Department of the Navy. b. Petitioner contended she had an undiagnosed medical condition which led to her failure of the Cycle 2, 2013 Body Composition Assessment (BCA). c. Enclosure (2), Petitioner's PRIMS PFA record, reflects Petitioner failed Cycle 2, 2013 Body Composition Assessment (BCA). Consequently, Petitioner was not authorized to participate in the Physical Readiness Test. d. Petitioner was issued enclosure (3), a Periodic/Regular evaluation for the reporting period 14 July 2013 to 15 September 2014. Block 20 ofthe evaluation report reflected that she failed Cycle 2, 2013 PFA. e. ln correspondence attached at enclosure (4), the Medical Corps officer treating the Petitioner stated that she had recently been diagnosed with a medical condition that can cause erratic weight gain and extreme difficulty losing weight, and she likely had this condition since at least June of 2013. Further, Petitioner's body composition analysis failure is related to this underlying medical condition. f. In correspondence attached at enclosure (5), Petitioner's Commanding Officer requested Petitioner's Cycle 2, 2013 Physical Fitness Assessment (PFA) be corrected to reflect that she was waived from that Cycle due to an underlying medical condition, which had been undiagnosed until the present time. g. In correspondence attached at enclosure (6), Petitioner's Commanding Officer submitted a supplemental chief evaluation report to Commander, Navy Personnel Command (PERS-32), correcting Block 20 to reflect Petitioner was waived from Cycle 2,2013 PFA cycle. h. In correspondence attached at enclosure (7), the office having cognizance over Petitioner's request to correct her PRIMS record commented that in order to update the Petitioner's PRIMS record for Cycle 2, 2013, all medical waivers must be issued on a NAVMED 6110/4 and requires two signatures from the Physician (military or civilian) recommending the waiver, AMDR physician and if the AMDR is the recommending physician, the AMDRs supervisor's signature is required. The AO concluded that the Petitioner 's request does not meet the requirements to remove her failure from the PRIMS database. CONCLUSION Upon review and consideration of all the evidence of record, and in light of enclosures (4) and (5), the Board concluded that Petitioner's request warrants relief. Although the AO at enclosure (7) states that Petitioner's request does not meet the requirements to remove her failure from the PRIMS database, the Board determined that enclosures (4) and (5) substantiated Petitioner's contention that she had an undiagnosed medical condition that precluded her from passing the PFA. The Board concluded that Petitioner's PRIMS record shall be corrected to reflect the Cycle 2,2013 PFA was medically waived. RECOMMENDATION In view of the foregoing, the Board recommends the following corrective action. Petitioner's naval record be corrected by changing the PRIMS record for Cycle 2,2013 PFA to reflect Petitioner was medically waived. 4. Pursuant to Section 6(c) of the revised Procedures of the Board for Correction of Naval Records (32 Code of Federal Regulations, Section 723.6(c)) it is certified that a quorum was present at the Board's review and deliberations, and that the foregoing is a true and complete record of the Board's proceedings in the above entitled matter. 5.
