Pursuant to the provisions ofreference (a) Subject, hereinafter referred to as Petitioner, filed enclosure (l) with this Board requesting, in effect, that the applicable naval record be corrected to show her Home ofRecord (HOR), on her DD Form 4/1 (Enlistment/Reenlistment Document Armed Forces ofthe United States) and her DD Form 196611 (Record ofMilitary ProcessingArmed Forces ofthe United States), was changed to reflect , California. 2. The Board, consisting of , reviewed Petitioner's allegations oferror and injustice on 29 September 2017 and, pursuant to its regulations, determined that the corrective action indicated below should be taken on the available evidence ofrecord. Documentary material considered by the Board consisted ofthe enclosures, naval records, and applicable statutes, regulations and policies. 3. The Board, having reviewed all the facts ofrecord pertaining to Petitioner's allegations of error and injustice, finds as follows: a. Before applying to this Board, Petitioner exhausted all administrative remedies available under existing law and regulations within the Department ofthe Navy. b. In correspondence attached as enclosure (2), the office having cognizance over the subject matter addressed in Petitioner's application has commented to the effect that the request has merit and warrants favorable action. CONCLUSION Upon review and consideration of all the evidence ofrecord, and especially in light ofthe contents ofenclosure (2), the Board finds the existence ofan injustice warranting the following corrective action. Petitioner began the process ofjoining the Navy in in March 2015, but had difficulties completing her physical at the MEPS , which she did not complete until 9 October 2015 because ofpre-existing medical conditions. Petitioner required additional medical consultations there in , and was also having financial problems so she temporarily moved in with her parents in . Petitioner, however, moved her household goods to a storage unit in the area where they are currently at the present time. RECOMMENDATION That Petitioner's naval record be corrected, where appropriate, to show that: The Record ofMilitary Processing -Armed Forces ofthe United States (DD Form 196611), executed on or about 10 March 2016 is modified to read block 4 (Home of Record Address) " vice The Enlistment/Reenlistment Document -Armed Forces ofthe United States (DD Form 4/1), executed on or about 11 March 2016 is modified to read block 3 (Home ofRecord) " " vice A copy ofthis Report ofProceedings will be filed in Petitioner's naval record. 4. It is certified that a quorum was present at the Board's revieTand deliberations, and that the foregoing is a true and complete record ofthe Board's procedings in the above entitled matter. 5.
