Pursuant to the provisions of reference (a) Subject, hereinafter referred to as Petitioner, filed enclosure (1) with this the Board for Correction of Naval Records (Board) requesting, in effect, that the applicable naval record be corrected to establish entitlement to an Enlistment Bonus Source Rate (EBSR). 2. The Board, consisting of reviewed Petitioner's allegations of error and injustice on 7 June 2019 and, pursuant to its regulations, determined that the corrective action indicated below should be taken on the available evidence of record. Documentary material considered by the Board consisted of the enclosures, relevant portions of the Petitioner's naval record, and applicable statutes, regulations and policies. 3. The Board, having reviewed all the facts of record pertaining to Petitioner's allegations of error and injustice, finds as follows: a. Before applying to this Board, Petitioner exhausted all administrative remedies available under existing law and regulations within the Department of the Navy. b. In correspondence attached as enclosure (2), the office having cogniz.ance over the subject matter addressed in Petitioner's application has commented to the effect that the request has merit and warrants partial favorable action. CONCLUSION Upon review and consideration of all the evidence of record, and especially in light of the contents of enclosure (2), the Board finds the existence of an injustice warranting the following corrective action: Petitioner signed an Enlistment Guarantee Annex "C" for the Submarine Electronic Computer Field (SECF 5YO) Program. Per the governing Active Duty Enlistment Bonus and Enlistment Loan Repayment policy, Petitioner's SECF SYO program entitled him to an EBSR in the amount of $5,000. The Petitioner was under the impression he was entitled to an EBSR in the amount of$12,000. This misconception occurred due to an erroneous EBSR monetary amount listed on Petitioner's Enlistment Bonus Statement of Understanding. Moreover, Petitioner's Enlistment Guarantee Annex "C" had an erroneous DD Form 4 dated 28 April 2015. Petitioner's Enlistment Guarantee Annex "C" should reflect DD Form 4 dated 18 March 2015 vice 28 April 2015. RECOMMENDATION That Petitioner's naval record be conected, where appropriate, to show that: Petitioner's Enlistment Guarantee (NAVCRUIT 1133/52) Annex "C" of the DD Form 4 dated 28 April 2015 is modified to reflect: Option 2: "$5,000", vice Option 2: "None". Petitioner's Enlistment Guarantee (NAVCRUIT 1133/52) Annex "C" is modified to reflect DD Form 4 dated "1 8 March 2015", vice "28 April 2015". A copy of this Report of Proceedings will be filed in Petitioner's naval record. 4. It is certified that quorum was present at the Board's review and deliberations, and that the foregoing is a true and complete record of the Board's proceedings in the above entitled matter. 5.
