Pursuant to the reference, Petitioner, a former enlisted Sailor, filed the enclosure with this Board, requesting to change his DD Form 214, Block 3, social security number (SSN). 2. The Board, consisting of , reviewed Petitioner’s allegations of error and injustice on 24 February 2020 and, pursuant to its regulations, determined that the corrective action indicated below should be taken on the available evidence of record. Documentary material considered by the Board consisted of the enclosure, relevant portions of the Petitioner’s naval records, as well as applicable statutes, regulations, and policies. 3. The Board, having reviewed all the facts of record pertaining to Petitioner’s allegations of error and injustice, finds as follows: a. Before applying to this Board, Petitioner exhausted all administrative remedies available under existing law and regulations within the Department of the Navy. b. Petitioner enlisted in the Navy and began a period of active duty on 15 October 1974. On 12 May 1976, Petitioner requested cancellation of his active duty agreement because he was dropped from advanced training in the nuclear power field. On 13 May 1976, Petitioner’s commanding officer endorsed the request citing Petitioner’s grades were below the minimum required and recommending discharge. On 10 June 1976, Chief of Naval Personnel approved the request for discharge. On 17 June 1976, Petitioner was discharged with an honorable characterization of service, a separation code “KFC,” a reentry code “RE-3K,” and a narrative reason for separation of “enlisted/inducted in error.” c. Petitioner asserts the SSN listed in Block 3 of his DD Form 214 is incorrect. While in the recruiter’s office, he was asked to phone home and get his SSN. His mother read it to him and no other proof was ever required. Recently, Petitioner realized that SSN (“ ”) belongs to his brother. Petitioner asserts that he needs the correction in order to obtain social security and Veteran benefits. In support of his petition, he attached copies of his social security card, birth certificate, driver’s license, and correspondence from the Social Security Administration. These records reflect that Petitioner’s correct SSN is “ .” CONCLUSION Upon review and consideration of all the evidence of record, the Board finds the existence of an error warranting relief. RECOMMENDATION In view of the above, the Board directs the following corrective action: That Petitioner’s naval record be corrected to reflect his true and correct social security number of “ .” That Petitioner be issued a new DD Form 214 reflecting his corrected SSN. That no further action be granted. That a copy of this report of proceedings be filed in Petitioner’s naval record. That upon request, the Department of Veterans Affairs is informed that Petitioner’s application was received by the Board on 6 June 2018. 4. It is certified that a quorum was present at the Board’s review and deliberations, and that the foregoing is a true and complete record of the Board’s proceedings in the above-entitled matter. 5.
