Pursuant to the provisions of reference (a) Subject, hereinafter referred to as Petitioner, filed enclosure (1) with this Board requesting, in effect, that the applicable naval record be corrected to show Petitioner’s Home of Record reflect . 2. The Board, consisting of reviewed Petitioner’s allegations of error and injustice on 6 August 2020 and, pursuant to its regulations, determined that the corrective action indicated below should be taken on the available evidence of record. Documentary material considered by the Board consisted of the enclosures, relevant portions of Petitioner’s naval record, and applicable statutes, regulations and policies. 3. The Board, having reviewed all the facts of record pertaining to Petitioner’s allegations of error and injustice, finds as follows: a. Before applying to this Board, Petitioner exhausted all administrative remedies available under existing law and regulations within the Department of the Navy. b. On 15 November 1983, Petitioner was born in . c. In June 2001, Petitioner graduated from high school from . d. Petitioner’s DD Form 1966 (Record of Military Processing – Armed Forces of the U.S.) signed on 22 June 2011, block 4 (Home of Record Address) shows a home of record of . e. Petitioner’s DD Form 4 (Enlistment/Reenlistment Document Armed Forces of the U.S. On signed on 22 June 2011, block 3 (Home of Record) shows a home of record of ., . f. On 4 October 2011, Petitioner entered active duty, and currently is on active duty without a break. g. On 20 October 2016, Petitioner was issued an voter registration card. h. In correspondence attached as enclosure (2), the office having cognizance over the subject matter addressed in Petitioner’s application has commented to the effect that the request has merit and warrants favorable action. CONCLUSION Upon review and consideration of all the evidence of record, and especially in light of the contents of enclosure (2), the Board finds the existence of an injustice warranting the following corrective action. The Board concluded that Petitioner‘s DD Form 1966 is correct, and an error was made when Petitioner’s DD Form 4 was created. Furthermore, Petitioner was from , , , and the address was essentially used as a place of convenience to facilitate his entry into military service. RECOMMENDATION That Petitioner’s naval record be corrected, where appropriate, to show that: Petitioner’s Enlistment/Reenlistment Document (DD Form 4/1) executed on 22 June 2011 Home of record (block 3) was ” vice “. Note: That any material or entries inconsistent with or relating to the Board’s recommendation be corrected. A copy of this Report of Proceedings will be filed in Petitioner’s naval record. 4.
