1.  Pursuant to the provisions of reference (a) Subject, hereinafter referred to as Petitioner, filed enclosure (1) with this Board requesting, in effect, that the applicable naval record be corrected to establish entitlement to an Enlistment Bonus for Source Rate (EBSR) and Enlistment Bonus for Shipping (EBSHP).                                  
                                              
2.  The Board, consisting of reviewed Petitioner’s allegations of error and injustice on 30 August 2019 and, pursuant to its regulations, determined that the corrective action indicated below should be taken on the available evidence of record.  Documentary material considered by the Board consisted of the enclosures, relevant portions of the Petitioner’s naval record, and applicable statutes, regulations and policies. 
 
3.  The Board, having reviewed all the facts of record pertaining to Petitioner’s allegations of error and injustice, finds as follows: 
 
     a.  Before applying to this Board, Petitioner exhausted all administrative remedies available under existing law and regulations within the Department of the Navy. 
 
     b.  In correspondence attached as enclosure (2), the office having cognizance over the subject matter addressed in Petitioner’s application has commented to the effect that the request has merit and warrants favorable action. 
                                
CONCLUSION 
 
Upon review and consideration of all the evidence of record, and especially in light of the contents of enclosure (2), the Board finds the existence of an injustice warranting the following corrective action:  Petitioner signed enlistment guarantee Annex “B” which provided entitlement to an EBSR and EBSHP; however, the DD Form 4 date was erroneously listed as 19 October 2017.  The DD Form 4 dated on Enlistment Guarantee must reflect the date of the Enlistment/Reenlistment Document (DD Form 4/1).  This error had impeded Petitioner from receiving entitlement to EBSR and EBSHP.      

RECOMMENDATION 
 
That Petitioner’s naval record be corrected, where appropriate, to show that: 
 
Petitioner executed an Enlistment Guarantee (NAVCRUIT 1133/52) Annex “B” is modified to reflect DD Form 4 dated 9 November 2017 vice DD Form 4 dated 19 October 2017.  
 
Petitioner is entitled to an Enlisted Bonus for Shipping in the amount of $5,000 for meeting requirements per Active Duty Enlistment Bonus and Enlisted Loan Repayment policy dated 8 February 2018.  Defense Finance & Accounting Service will audit to ensure that the payment has not been previous awarded to the Petitioner. 
 
Note:  Petitioner successfully completed training requirements and is entitled to the Enlistment Bonus Source Rating in the amount of $10,000.  will process Petitioner’s payment for the Enlistment Bonus. 
                                                             
A copy of this Report of Proceedings will be filed in Petitioner’s naval record.
