Pursuant to the reference , Petitioner, a former naval officer, filed the enclosure with the Board for Correction of Naval Records (Board) requesting to change his first name in Block 1, of his DD Form 214. 2. The Board, consisting of , reviewed the Petitioner’s allegations of error and injustice on 7 November 2019, and pursuant to its regulations, determined that the corrective action indicated below should be taken on the available evidence of record. Documentary material considered by the Board consisted of the enclosures, relevant portions of the naval records, and applicable statutes, regulations, and policies. 3. The Board, having reviewed all the facts of record pertaining to Petitioner’s allegations of error and injustice, finds as follows: a. Before applying to this Board, Petitioner exhausted all administrative remedies available under existing law and regulations within the Department of the Navy. b. Petitioner enlisted in the Navy and began a period of active duty on 29 May 1981. He signed his service agreement (dated 30 October 1980) as . On 31 March 2001, Petitioner retired and his DD 214 lists his name as . c. Petitioner entered the service in 1981 using his nickname, , which at the time was on his social security card. Since his discharge, Petitioner was required to change my name with the Social Security Administration so that it matches the name on is birth certificate, . In support of his application, Petitioner attached copies of his birth certificate; passport, driver’s license, and social security card which all list his first name as . He also included his retired military identification card, which lists the first name of . CONCLUSION Upon review and consideration of all the evidence of record, the Board finds the existence of an error warranting relief. The Board decided that it was an error to have the first name instead of on Petitioner’s DD 214 and that it should be corrected. RECOMMENDATION In view of the above, the Board directs the following corrective action: Petitioner’s naval record be corrected by changing Block 1 on his DD Form 214 from to . Petitioner be issued a new DD Form 214. That no further action be granted. A copy of this report of proceedings be filed in Petitioner’s naval record. 4.
