Pursuant to the provisions of reference (a) Subject, hereinafter referred to as Petitioner, filed enclosure (1) with this Board requesting, in effect, that the applicable naval record be corrected to receive Involuntary Separation Pay (ISP) and DD 214 correction. 2. The Board, consisting of , reviewed Petitioner’s allegations of error and injustice on March 27, 2020 and, pursuant to its regulations, determined that the corrective action indicated below should be taken on the available evidence of record. Documentary material considered by the Board consisted of the enclosures, relevant portions of Petitioner’s naval record, and applicable statutes, regulations and policies. 3. The Board, having reviewed all the facts of record pertaining to Petitioner’s allegations of error and injustice, finds as follows: a. Before applying to this Board, Petitioner exhausted all administrative remedies available under existing law and regulations within the Department of the Navy. b. On May 8, 1989, Petitioner enters active duty. c. On April 1, 1993, Petitioner released from active duty on Early Separation Program and transferred to Inactive Reserve. d. On April 7, 1995, Petitioner enlist in Naval Reserve for 4 years. e. On September 19, 1995, Petitioner request discharge from delayed entry program and enlist in Regular component of Navy for four (4) years. f. On August 10, 2000, Petitioner discharged, Narrative reason for separation (Condition, not a disability). Character of Service (General (Under Honorable Conditions)). Separation Code (GFV). Block 18 of DD Form 214 states (Disability severance pay authorized in amount of $4,303.53 but not paid). g. On November 12, 2003, Petitioner’s record was corrected with issuance of DD Form 215. Block 12a. Date Entered AD this Period (95 Sep 19); Block 12c. Net Active Service this Period (04 10 22); Block 12d. Total Prior Active Service (03 10 24). No further entries corrected. h. On March 10, 2020, Defense Finance and Accounting Service (DFAS) representative from the Corrections of Records/Claims confirmed ISP payment in the amount of $8,367.97, net payment $6,814.72 after Federal and State deductions was paid to the Petitioner on August 21, 2000, voucher number AA000647. CONCLUSION Upon review and consideration of all the evidence of record, and especially in light of the contents of enclosure (2), the Board finds the existence of an injustice warranting the following partial corrective action. RECOMMENDATION That Petitioner’s naval record be corrected, where appropriate, to show that: DD 215 issued to correct DD Form 214 issued on August 10, 2000. Block 18 (Remarks), “Disability Severance Pay Authorized in amount of $8,367.94, ($6,814.72 paid) voucher AA000647.” A copy of this Report of Proceedings will be filed in Petitioner’s naval record. 4. It is certified that quorum was present at the Board’s review and deliberations, and that the foregoing is a true and complete record of the Board’s proceedings in the above entitled matter. 5.
