Pursuant to the provisions of reference (a) Subject, hereinafter referred to as Petitioner, filed enclosure (1) with this Board requesting, in effect, that the date of entry and rank be corrected on his Certificate of Release or Discharge from Active Duty (DD Form 214). 2. The Board, consisting of , reviewed Petitioner’s allegations of error and injustice on 24 March 2020 and, pursuant to its regulations, determined that the corrective action indicated below should be taken on the available evidence of record. Documentary material considered by the Board consisted of the enclosures, relevant portions of Petitioner’s naval record, and applicable statutes, regulations and policies. 3. The Board, having reviewed all the facts of record pertaining to Petitioner's allegations of error and injustice, finds as follows: a. On 16 June 1960, Petitioner entered the Navy Reserve. b. On 27 June 1960, Petitioner transferred to Officer Candidate School (OCS) on active duty. c. On 13 October 1960, Petitioner was discharged. d. On 14 October 1960, Petitioner was commissioned an ensign/O-1 in the Navy Reserve and entered active duty. e. On 1 October 1964, Petitioner was promoted to lieutenant/O-3. f. On 14 October 1964, Petitioner was released from active duty and transferred to the Navy Reserve. g. On 28 April 1975, Petitioner was discharged. h. Before applying to this Board, Petitioner exhausted all administrative remedies available under existing law and regulations within the Department of the Navy. CONCLUSION Upon review and consideration of all the evidence of record, the Board agreed that Petitioners date of entry and rank was in error. The Board finds the existence of an injustice warranting the following corrective action. RECOMMENDATION That Petitioner’s naval record be corrected, where appropriate, to show that: Petitioner’s DD Form 214 block 3a [GRADE] changed to “LT” vice “LTJG”. Petitioner’s DD Form 214 block 3b [DATE OF RANK] changed to “1 October 1964” vice “14 April 1962”. Petitioner’s DD Form 214 block 19c [DATE OF ENTRY] changed to “14 October 1960” vice “14 October 1964”. A copy of this Report of Proceedings will be filed in Petitioner’s naval record. 4. It is certified that quorum was present at the Board’s review and deliberations, and that the foregoing is a true and complete record of the Board’s proceedings in the above entitled matter. 5.
