Pursuant to the provisions of the reference, Petitioner, a former enlisted Sailor, filed the enclosure with this Board, requesting changes to his DD Form 214, Block 24, Decorations, Medals, Commendations, Citations, and Campaign Ribbons Awarded or Authorized. 2. The Board, consisting of , reviewed Petitioner’s allegations of error and injustice on 9 March 2020 and, pursuant to its regulations, determined that the corrective action indicated below should be taken on the available evidence of record. Documentary material considered by the Board consisted of the enclosure, relevant portions of the Petitioner’s naval records, and applicable statutes, regulations, and policies. 3. The Board, having reviewed all the facts of record pertaining to Petitioner’s allegations of error and injustice, finds as follows: a. Before applying to this Board, Petitioner exhausted all administrative remedies available under existing law and regulations within the Department of the Navy. b. Petitioner enlisted in the Navy and began a period of active duty on 8 October 1964. On 9 November 1968, Petitioner was discharged with an honorable characterization of service within three months of expiration of his contract. On 7 October 1970, Petitioner was discharged with an honorable characterization of service and recommended for reenlistment from the Navy Reserve. On 31 January 1973, Petitioner was discharged with an honorable characterization of service and recommended for reenlistment from the Navy Reserve. c. Petitioner’s DD Form 214, Block 24 indicates he is entitled to the following: National Defense Service Medal (NDSM), Vietnam Service Medal (VSM), Vietnam Campaign Medal (VCM), and Good Conduct Medal (GCM). d. Petitioner asserts that pursuant to the 22 November 2010 Review of Record (NAVPERS 1650/96) he is entitled to those awards already listed on his DD Form 214 and the following: Navy/USCG Unit Commendation Medal (NUC) Combat Action Ribbon (CAR) MUC (Gallantry Cross w/Palm and Frame Ribbon Bar) MUC (Civil Action Color w/Palm and Frame Ribbon Bar) Service Medal w/ 4 Bronze Stars. CONCLUSION Upon review and consideration of all the evidence of record, the Board finds the existence of an error warranting relief. RECOMMENDATION In view of the above, the Board directs the following corrective action: Petitioner’s naval record be corrected by documenting all the decorations Petitioner is entitled to. Petitioner be issued a DD Form 215. That no further action be granted. A copy of this report of proceedings be filed in Petitioner’s naval record. That upon request, the Department of Veterans Affairs is informed that Petitioner’s application was received by the Board on 30 July 2019. 4. It is certified that a quorum was present at the Board’s review and deliberations, and that the foregoing is a true and complete record of the Board’s proceedings in the above-entitled matter. 5.
