Pursuant to the provisions of reference (a), Subject, hereinafter referred to as Petitioner, filed enclosure (1) with the Board for Correction of Naval Records (Board), requesting adjustment to the his Certificate of Release or Discharge from Active Duty (DD Form 214) to reflect his honorable period of service from 26 March 2015 to 4 March 2019. Enclosures (1) through (3) apply. 2. The Board, consisting of Mr. , Ms. , Mr. , reviewed Petitioner's allegations of error and injustice on 23 July 2021, and, pursuant to its regulations, determined that the corrective action indicated below should be taken on the available evidence of record. Documentary material considered by the Board consisted of the enclosures, relevant portions of his naval service records, and applicable statutes, regulations, and policies. 3. The Board, having reviewed all the facts of record pertaining to Petitioner's allegations of error and injustice finds as follows: a. Before applying to this Board, Petitioner exhausted all administrative remedies available under existing law and regulations within the Department of the Navy. b. Although enclosure (1) was not filed in a timely manner, it is in the interest of justice to review the application on its merits. c. The Petitioner enlisted in the Navy and began a period of active duty on 26 March 2015. On 5 March 2019, Petitioner reenlisted for a period of four years. On 5 March 2020, Petitioner was notified of the initiation of administrative separation proceedings by reason of alcohol rehabilitation failure. On 9 March 2020, Petitioner elected his right to consult with counsel. On 6 April 2020, Petitioner was discharged with a general characterization of service due to alcohol rehabilitation failure. d. Petitioner’s DD Form 214 does not include his period of honorable service from 26 March 2015 to 4 March 2019. Per Reference (b), Box 18 should indicate Petitioner’s periods of honorable service. e. Petitioner provides excerpts from his OMPF to support his request of adjustment to his discharge documents. CONCLUSION: Upon review and consideration of all the evidence of record, and in light of reference (b), the Board determined Petitioner’s record warrants relief. The Board noted Petitioner’s Certificate of Release or Discharge from Active Duty (DD Form 214) does not indicate his period of honorable service from 26 March 2015 to 4 March 2019. In view of the above, the Board directs the following corrective action. RECOMMENDATION: In view of the above, the Board directs the following corrective action: Petitioner be issued a Correction to Certificate of Release or Discharge from Active Duty (DD Form 215), per reference (b) to add indication of Petitioner’s continuous honorable service from 26 March 2015 to 4 March 2019. That a copy of this report of proceedings be filed in Petitioner’s naval record. 4. It is certified that a quorum was present at the Board’s review and deliberations, and that the foregoing is a true and complete record of the Board’s proceedings in the above entitled matter. 5.
